The Women's Health and Cancer Rights Act of 1998 requires that if all or part of a breast is surgically removed and the removal is an eligible charge under the Plan, eligible charges related to the mastectomy include:

• Reconstruction of the breast on which the mastectomy was performed;
• Surgery and reconstruction of the other breast to provide a symmetrical appearance;
• Protheses; and
• Treatment of physical complications at all stages of the mastectomy, including lymphedemas.
