Please obtain a form for making changes from your Employer and notify us promptly in writing of any change of address. Also, it is very important that we are notified immediately as to any change in your family status, such as marriage, divorce, birth of a child, Adoption or Placement for Adoption of a Dependent child, legal Custody of a Dependent child, Qualified Medical Child Support Order for a Dependent child, marriage of any Dependent children, death of any Dependent, or loss of Dependent status on any Dependent due to age, marital, or student status as defined on your Schedule of Benefits.
