HOW TO OBTAIN A CERTIFICATE OF CREDITABLE COVERAGE:

According to the requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPPA), a Certificate of Creditable Coverage will automatically be provided to a Covered Employee when the Covered Employee or a Covered Dependent ceases to be eligible for benefits under the Employee Health Benefit Plan. Certificates of Creditable Coverage will also be provided upon request while a Covered Employee or a Covered Dependent is covered under the Employee Health Benefit Plan and up to twenty-four (24) months after coverage under the policy ends. You may obtain a Certificate of Creditable Coverage by calling Phillips Administrative Service, Inc., at 1-800-356-9822 or by sending a written request to: Phillips Administrative Service, Inc., P.O. Box 218, Albany, OR 97321.

(In the case of conflicting information, your Schedule of Benefits takes precedence.)
