ELIGIBILITY DATES OF COVERAGE

EMPLOYEE COVERAGE: An Employee becomes eligible for Coverage provided by this Plan on the later of:

  1. the Plan's Effective Date; or
  2. the completion of the Waiting Period as specified on your Schedule of Benefits.

DEPENDENT COVERAGE: An Employee becomes eligible for Dependent Coverage on the later of:

  1. the date the Employee becomes eligible for Coverage; and
  2. the date the Employee first acquires a Dependent; or
  3. the date a Qualified Medical Child Support Order is put in place.

WHO IS TO BE COVERED: If Employer pays 100% of premium, all eligible persons must be covered for life insurance, unless waived in writing for religious reasons. Eligible persons may opt, in writing, to waive medical coverage for themselves and their Dependents. If Dependents are to be covered, an eligible Employee must also be covered. If an eligible Employee is waiving Employee or Dependent coverage, the reason for waiving must be stated on the written waiver.

(In the case of conflicting information, your Schedule of Benefits takes precedence.)
